AUSTRALIA -- Australia is introducing a new program to improve the health of war veterans and their widows who have complex care needs and at risk of hospitalization.
The innovative Coordinated Veterans' Care (CVC) program is set to start on May 1 and Bupa Health Dialog, the Australian arm of Health Dialog, has been selected as the primary service provider.
"The signing of contracts with these organizations means we're on track to introduce a new health care program for our most vulnerable veterans and war widows," said Warren Snowdon, Minister for Veterans' Affairs.
Health Dialog is a wellness, prevention and chronic disease management company with expertise in analytics. In the CVC program, it will analyse client data to identify and contact veterans and war widows who are DVA Gold cardholders and are most likely to benefit from taking part in the CVC Program.
Snowdon said in the future, the Personally Controlled Electronic Health Record system will also support the CVC Program.
