Hong Kong's Hospital Authority (HA) published two regular bulletins to promote risk management among healthcare professionals in public hospitals on Friday.
"HA Risk Alert" collates reports on patient safety cases and is distributed among healthcare professionals. The "Medication Safety Bulletin," on the other hand, features regular patient cases in support of the continuous quality improvement initiative.
The Hong Kong government website said in the report that the latest HA Risk Alert reports on a case of inadvertent reduction of drug dosage, a case of retained gauze and six cases of patient suicide. Meanwhile, in the latest issue of the Medication Safety Bulletin, six cases for sharing with recommendations were featured to prevent recurrence of incidents of a similar nature.
HA Chief Executive Dr. Leung Pak-yin said that starting January 1, 2010, hospitals were required to report serious untoward events.
"The main purpose is to mitigate risks and improve patient safety by further enhancing staff awareness to clinical risk and stimulate innovative thinking for safe designs, processes and practices," he said.
